  108.  Now then, you can if you wish (and of course if you have a printer) print out 
  109. this questionnaire and fill it out or you can load up this file into your word
  110. processor and edit it  accordingly.

  112.  I would  much preffer  you save this file to disk and send that as it'll make 
  113. my  life  a whole  lot easier. Having said that, either  way is  fine with  me 
  114. however, PLEASE DO send  in your completed questionnaire as soon  as possible, 
  115. don't think of this as  a waste  of time, I REALLY DO NEED YOUR OPINIONS...!!

  118. inclusion...as long as you tell us about them....
